For me, it has always been a nightmare to do so. I hope other will find it useful.

I always had trouble with this and I since I didn't like polluting theme icons and the added overhead (I have to always move manually to a protocol and so on), so for this I've created IconFactory ( http://smalltalkhub.com/#!/~peteruhnak/IconFactory ) that I am using instead.
